Registration Form for Students - Use this form when registering with Disability Services.
Release of Information Form for Students - Use this form when allowing physicians, health care providers, or other agencies to release information to Disability Services.
Request for Exam Accommodations Form - Use this form when requesting accommodations for an exam. This form must be completed for each exam on which you are requesting accommodations.
