Copies of this form will be sent to the USD Greek Life Advisor and then distributed to each fraternity chapter. By submitting this form, I give permission to release my academic record to the IFC Recruitment Committee and Chapter Presidents for use in the membership selection process. If I accept a fraternity bid, I give permission to release my semester and cumulative GPA to the President, Scholarship Chair, Chapter Advisor, Live-in Graduate Academic Advisor of my fraternity and USD Greek Life Advisor.
