Copies of this form will be sent to the USD Sorority and Fraternity Life Advisor and then distributed to each sorority chapter. By submitting this form, I give permission to release my academic record to the PHC Recruitment Committee and Chapter Presidents for use in the membership selection process. If I accept a sorority bid, I give permission to release my semester and cumulative GPA to the President, Scholarship Chair, Chapter Advisor, and USD Sorority and Fraternity Life Advisor.
