Acceptance into our program is based on income eligibility.  We need some form of income verification with your application, which can be the previous year's income tax form, W2 form, pay stubs, financial aid award, TANF letter, etc.

To complete the enrollment process, we need the following information:
  • Completed application
  • Family's proof of income
  • Child's birth record
  • Child's immunization record
  • Dental/Oral health exam (within 45 days of enrollment or written proof)
  • Well baby/Well child exam (within 45 days of enrollment or written proof)
  • If you have more than one applicant, please submit a separate application for each.
