Authorization Form
Authorizes USD to use Title IV funds for charges other than those of the current semester's tuition.

2011-12 Verification Form
For students selected for verification for the 2011-12 academic year, complete and return this form with any requested documents. 
 
Loan Cancellation or Reduction Form
For students needing to cancel or reduce a Stafford, Grad PLUS, Parent Plus or alternative loan credited to their tuition account.

Parent Plus Request 2011-12
Parents - use this form to apply for a 2011-12 Parent PLUS Loan.

Grad Plus Loan Request
Graduate students - use this form to apply for a Graduate PLUS Loan.

Loan Disbursement and Student Liability Form
Form required before the distribution of loans for non-standard terms and summer loans. 

Satisfactory Academic Progress Appeal Form
Students who have had their financial aid suspended due to unsatisfactory academic progress may appeal to the Office of Student Financial Aid by submitting the appeals form within 30 days of notification of unsatisfactory status, and must be accompanied by all relevant documentation.
