USD Graduate School Application and Fee for the PA Program

In addition to the CASPA online application, all applicants are required to apply to The University of South Dakota Graduate School and pay a $35.00 application fee. You must submit the application and fee by the appropriate deadlines.

*Please note-this process needs to be done using a PC, not a Mac.

  1. Complete the USD Graduate School Health Sciences Application
  2. Save it to your computer titled "PA_2015lastname" (ex. for John Doe-"PA_2015Doe")
  3. Once you've completed the USD Graduate School Health Sciences Application, upload your application to the USD Graduate School
  4. Next, choose "Upload Health Sciences Application" and upload your application
  5. Finally, choose your form of payment for the application fee. You may pay online with a credit/debit card (via secure web page) or you may mail in a check.
