Action! Forms
The IdEA Program provides the following forms to assist you in completing and properly documenting your Action component. This process allows you to be ready to engage in your Action component, having thought about how it relates to your IdEA course and what you expect to learn from the experience. Download all the required Action forms (approval, release, time/effort). Please follow the guidelines, as well. If you need help completing the forms, stop in the Center for Academic Engagement in Old Main 103, email engage@usd.edu, or call 677-6338.

Required Forms: Action Guidelines ( PDF) - please read and review Action Guidelines prior to submitting the Action Approval form
Action Release Form ( PDF) - should be completed prior to beginning Action
Action Time/Effort Report Form ( PDF) - used to record time during Action work
Action Approval Form ( PDF) or Submit your Action Approval Form online (research and creative activities must be submitted in hard copy)
**NOTE regarding submission of Action Approval on line** You must be thorough when explaining how your Action relates to your theme. Failure to do so may prevent your Action from being approved. Submission does not guarantee approval. An e-mail regarding your submission will be sent to you from an Action Officer in the Center for Academic Engagement. If you have not received a response about the status of your approval within two business days, please contact the Action Officers. Creative activities and research projects require a sponsor's signature and therefore cannot be submitted online.

Reflection Suggestions: Students taking capstones until Fall 2009 may complete the Action component anytime from the start the foundation class until midway through the capstone class. Students seeking assistance in remembering details about their Action component to use that information in their capstone coursework may download the following sheet which will help record those experiences. Reflection Rubric ( PDF): This form is a suggested rubric for the Action reflection paper. However, not every professor uses this rubric, so check please check the course syllabus.
