health insurance

Every international student is required to have health insurance. All international students must buy health insurance for themselves and accompanying  dependents for the entire duration of their stay.  This is a Board of Regents policy which cannot be waived unless the student's insurance is paid for by his/her government or sponsoring agency.

The Procedure
1. Go to the Student Health Clinic first.

• Student Health is located at 20 S. Plum Street in Sioux Valley Hospital in Vermillion.  The phone number for that office is 624-9111.  They provide good quality medical care at a low cost.  If Student Health cannot help you, they will refer you to another doctor.
• Get a referral from the Student Health Clinic before you go to a different doctor.
• Exceptions: do not go to the Student Health Clinic first if

                                *The Student Health Clinic is closed
                                *You have an emergency
                                *You are visiting another city

• The U health insurance plan works at all hospitals and clinics in the U.S.

2. Send a claim form to the insurance company

• Most hospitals and doctors complete the claim for you
• Claim forms are available in the Student Health Clinic at 20 S. Plum Street.  We can help you fill out the claim form.  Then, send the claim form and a copy of your medical bill to the insurance company.

3. The insurance company will send you an EOB (Explanation of Benefits) in the mail.

• The EOB tells you how much of your medical bill the insurance company will pay.
• Sometimes the insurance company does not pay the entire amount.  You pay the remaining amount to the hospital.
• It usually takes 4-6 weeks for the insurance company to process your claim form, sometimes longer.

4. If you have any questions about student health insurance, call Laura Crosley at (605) 624-9111 or stop by the Student Health Insurance Office at 20 S. Plum.

Helpful Facts:
All F-1  international students and their families must purchase the USD health insurance plan.

• The insurance company pays for doctor and hospital visits that are medically necessary, like accidents and sicknesses.
  Dental work is not covered, unless you accidentally hurt a healthy tooth.
  Vision care is not covered, unless you accidentally hurt your eye.
• Pregnancy is only covered if you become pregnant while you have the USD health insurance plan.  Pregnancy coverage is limited.
• Allergy treatments and cosmetic treatments are not covered.

Your insurance plan has a $200 deductible.
You must pay the first $200 of your medical bills.  You only have to pay the $200 deductible once per year.

Payment Options
1. Pay by semester: Fall Semester (August to January)
   Spring/Summer combined(January to August)

2. Pay one annual fee for the entire school year (This is a bit cheaper)
