admission procedure
1. Request a USD application from the USD Admissions Office (1-605-677-5434 or 1-877-269-6837). Follow all instructions and return the application to the address listed. 2. In order to be considered for the USD Nursing program, you must be accepted to USD by January 1st if you are applying for the fall admission to nursing classes OR July 1st if you are applying for spring admission to nursing classes. (See below)
Enrollment at each campus is limited. Application materials received after the deadlines will be considered on a space available basis. You will be notified in writing by the Department of Nursing regarding your admission status. 1. Download USD Department of Nursing Application forms. Application forms include: application, Physical and Mental Requirements, Plan of Study and three reference forms. You will need Acrobat reader to get the forms. 2. Transcripts
Complete and return the nursing application to the USD Department of Nursing campus to which you are applying.
