2009 Summer Financial Aid Request Form: Use this form to request a Stafford loan, an alternative loan or summer work study. Satisfactory Academic Progress Form: To appeal, complete this form and return to the USD Financial Aid Office. Verification: If you have been selected for the verification process for the 2009-2010 school year, please submit the requested documents to the Enrollment Service Center (ESC) as soon as possible. Parent Plus Loan Request Form: Parents, you can submit a request for a Parent Plus Loan for your dependant undergraduate student with this form. Apply for the Grad PLUS: Graduate students, to apply for the Graduate PLUS loan, print this form and return it to the Financial Aid Office.
