Additional Required Information
Three references (work or education-related)

Students enrolled in a RN-BSN nursing program other than USD are eligible for selection into the USD nursing program as long as they have left the previous program in "good standing." One reference must be from the previous nursing chairperson, director or designee.
**It is the responsibility of the applicant to ensure all references are submitted on or before due date.**

Your application is considered "complete" only when ALL required items are received. If you have questions regarding the application or the applications process please call 605-658-5600, email Beth Melstad at or contact your adviser.

There is no fee to submit your application. However, if you are admitted to the RN-BSN program and accept the conditional offer of admission, you will be required to complete a background check and drug screen through Verified Credentials, Inc., submit a deposit to hold your position in the program, as well as become compliant with all USD Nursing & Health Sciences requirements (immunizations, CPR, physical exam, etc.). The applicant will be responsible for all expenses incurred. See Nursing Terms of Acceptance.
