The TransitionInAction Clinic supports adolescents with disabilities as they transition to adulthood. Our goal is to positively impact the post-school outcomes and quality of life experienced by these adolescents, especially in integrating education, health, employment and independent living.

We use interviews, assessments, activities and discussions that allow our team to get to know the young adult’s strengths, interests and vision for the future. Together with the adolescent’s family, the multidisciplinary clinic team creates a comprehensive plan for transition that is focused, coordinated and action-oriented, all at no cost to the person.
